It depends.
For me, I feel like I often decide as I'm writing the lyrics. They often come to mind just after the first chorus.

Of course.

That is TOMO, but the one wearing plaid is HARU. She's wearing the same things as she is in this picture.

You're right, Haru's hands look gigantic in that pic. They all have really small hands though..
